Holy Name Parish School Application Information
New students only. Students currently enrolled in Holy Name do not need to re-register.
Nursery School
Child must be (3) years old by September 1
Kindergarten I
Child must be four (4) years old by September 1
Kindergarten II
Child must be five (5) years old by September 1
Nursery & New Families Only
$100 non-refundable registration fee is due at registration.
The following documents are required to complete the application process:
1 Application Form for appropriate grade
2 Birth Certificate–copy
3 Baptismal Certificate–copy
4 Copy of report card (Grades Kindergarten II–Grade 6)
5 Registration fee as noted above
6 Parish envelope to verify in/out parish status (Grades 1-6)
7 City of Boston Public Health Commission School Health Program Health
History for New Enterers Form
8 Health records (immunizations)
9 Principal Recommendation form (Grades 1–5)
10 Signed CORI form(s) for both parents (must show license for verification)
11 IRIS–Immediate Response Information System Form
In order to receive In-Parish tuition rates, families must attend weekly Mass at Holy Name Parish and use the offertory envelopes.
Upon acceptance a $500 non-refundable deposit is required; this will be applied to tuition.
